By submitting this entry form I understand all coaches must be USBC RVP representatives or a registered volunteer for another youth organization. All coaches must be SafeSport certified and can provide documentation upon request for both certifications to confirm.
Coaches without these credentials can still support their teams from the spectator area.
The following information must be emailed to Tournament Director within 15 days of entry submission -
All Bowlers must provide proof of School attending. We will accept student ID, report card or class schedule for validation. Items must have bowlers name and school name clearly printed. This information must be emailed to Tournament Director within 15 days of submitting entry.
Teams must show proof of existing team structure prior to acceptance in event. All teams must provide current league standings sheet showing date of standings, league name and all team information for entrants. This information must be emailed to Tournament Director within 15 days of submitting entry.
Bowling order will be entered based on the order of the bowlers below. Place any substitute bowler at the bottom of list.
Bowlers must be 10 years of age by August 1, 2023.
Teams are required to have 4 bowlers. Up to 5 bowlers are allowed due to substitutions. Bowler 5 is optional.
